Your First Fertility Consultation: What to Expect

Taking the step toward a fertility consultation can be stressful. At CRE, we understand how difficult this journey can be and are here to support you every step of the way.

Here’s how to prepare and what to expect:

  1. Complete Your Paperwork in Advance
    • Once you have scheduled an appointment, the receptionists will e-mail you a welcome package that includes important forms. It is important to fill out and email them at least 48 hours before your appointment to CRETXPatientServices@dallasfertilitycenter.com
    • This ensures we can review your information ahead of time, request any insurance authorization if needed and help you maximize your time with the physician. Filling these forms in the waiting room would be stressful and delay your visit significantly.
  2. Bring the Essentials
  3. Please bring the following to your appointment:

    • A valid photo ID
    • Your insurance card
    • Any relevant medical records (previous fertility treatments, lab results, imaging, embryology reports etc.). For previous HSGs, it is very helpful to bring copies of the actual images, not just the report.
    • A credit card, as CRE operates in a cashless environment. All consultation and treatment-related charges are due at the time of service.
  4. Meet Your Care Team
    • Your appointment will be with either Dr. Saleh or Dr Collins, both Board certified REI known for their compassionate and personalized care. A routine pelvic ultrasound is not necessary unless there is known issue that requires follow-up.
    • Skilled support staff will walk you through the diagnostic, treatment, and insurance processes as soon as your doctor and you have agreed on a specific treatment plan.

    • Warm, attentive care
    • Guidance through each step of the process – each IVF patient is assigned a personal nurse.
    • A collaborative approach—understanding your needs, goals, and circumstances before recommending a treatment.

Rapid Fertility Testing

Tired of having to waste so much time scheduling visits and appointments? Consider our RFT option and have all your tests in one visit. The results and treatment plan can then be discussed a week later.
